Dear Sir or Madam

I have my car insured in your firm since 1997. Recently I had a car accident, and I'm writing to you to have the costs of repairing damages covered.

The accident happened on 5 December 2000 near the Warsaw on the E345 road to Legionowo. I was driving north during the storm and a tree fell on my car. Fortunately I survived the crash with only minor injuries.

The car is in poor condition: the ceiling is smashed into the interior, all windows are broken, the chairs have been damaged, and the reflectors have been destroyed. The vehicle will surely need general renewal, including installing new engine and repainting whole body.

The car has been already taken to the parking lot, and waits there till your specialist sees it. Because of that, I kindly request to have another car assigned during the time of repair, as my job involves moving to a long distances.

I also want to know what exactly are procedures in such cases, and what further actions should I take in order to receive compensation.
